Take Charge of Your Health
Intended for individuals diagnosed with diabetes, high blood pressure, high cholesterol, weight issues, COPD, depression, or chronic pain, this series is based on the Chronic Disease Self-Management Program developed by Stanford University. Research has demonstrated the effectiveness of the program in improving health outcomes of participants over time. Topics to be covered include: coping with fatigue, frustration and pain, exercising to maintain and improve strength, using medications, practicing good nutrition, evaluating new treatment options.
